Incident Narrative

On September 24, 2015, at approximately 11:15 a.m., an employee's middle fingertip became caught inside the pin hole of a crane that was being used to set up temporary lighting. The employee was coming off the ladder and used the pin hole for leverage. The crane operator thought he was clear to actuate the boom, and while doing so the employee's fingertip was amputated.
