Incident Narrative

An employee was approximately 16 feet aboveground sitting on a steel horizontal beam. He was installing bolts to replace the studs that he connected to the holders. As another beam was being drifted in by the crane, the beam that he was sitting on became dislodged and fell to the ground along with the employee.
