Incident Narrative

A flooring employee was cleaning an area on the fourth floor of a new construction project. The employee lifted a piece of plywood that was covering a floor opening and then fell through the hole to the third floor. The employee fell approximately 16 feet, fracturing his lower back and tail bone areas.
