Incident Narrative

An employee opened the gate to inspect a part of the Tangent deep draw hydraulic press. Lock out/tag out wasn't used. A co-worker provided additional light and crossed the light curtain in front of the press activating it. The injured employee suffered cuts to both arms, burns to the left hand and a compression injury to both arms.
