Incident Narrative

An employee was machining a packing tube when a bushing on the bar came loose. The employee attempted to push the bushing back in place and his left hand got caught in the rotating bar, amputating his left index finger and fracturing the adjacent fingers. The employee was wearing gloves at the time of the incident.
