Incident Narrative

An employee was changing a swab on a mud pump using a wrench to stroke the pump. The pipe wrench slipped and he fell backwards off the skid to the ground. His back and head struck the ground and he injured his chest. He sustained a head laceration, upper right bicep cut, chest contusion, and concussion.
