On September 7, 2015, at about 04:00 PM, an employee fell from a 4- to 5-foot scaffold while cutting an aluminum structure beam with a chain saw. The saw kicked back, causing the employee to lose his balance and fall to the concrete floor. He sustained a head laceration and was hospitalized.
