Incident Narrative
An employee was preparing to run a new cable line from a utility pole to a house. The employee leaned a 28-foot ladder against the cable strand and was climbing or working when the staple fastener that was holding a strand support came out of the pole. The strand then fell 4 feet and the employee fell from the ladder, sustaining a serious head injury and other broken bones.
