Incident Narrative
On August 31, 2015, at approximately 11:30 a.m., an employee was helping a concrete pumper during backup to position the concrete chute for pouring. As the truck was reversing with chute extended, it appeared as if the chute was going to strike against a switch gear unit. The employee attempted to push the chute to give it more clearance, but the chute was locked in position and wouldn't move. His finger got pinched between the chute and the switch gear unit, and his left pinky fingertip was amputated. He was hospitalized for surgery.
