Incident Narrative

An employee was backing a forklift off a scale. He backed over a wooden pedestrian walkway that gave way under the weight of the machine, tipping the forklift onto a column. The employee's left hand was trapped between the column and the forklift crushing three fingers. The employee was hospitalized.
