Incident Narrative
An employee pulled Printing Section # 1 out of the Martin Machine and opened the interlocked cover on the ink rollers. He then took a pair of vise-grips, depressed the jog button, and clamped the button to the housing in the depressed position causing the rollers to rotate slowly. The employee then used a hose to spray water on the partially submerged ink rollers to clean them. The inside of the container in which the rollers were partially submerged was painted black. The color of the ink was also black. The employee reached his right hand into the liquid to see if it was clear, and his hand was pulled into the nip points of the rollers resulting in the amputation of the middle and ring fingers and one half of the index finger on his right hand.
