Incident Narrative

An employee was using a forklift to line up the shaft and bearing on a yarder. His finger was inside a bolt hole on a cable drum when the weight was shifted to line up the equipment. His finger got caught between the drum and the forklift fork, necessitating the amputation of his right index fingertip.
