Incident Narrative
On August 18, 2015, at approximately 7:00 p.m., two employees were performing die setting operations. The first employee used a forklift to place a die in a press. The second employee approached the backside of the press and began to install bolts in the die. The first employee had not finished aligning the die. When the first employee repositioned the die for bolting, it caused the washer and bolt that the second employee was holding to move and pinch the second employee's finger against the die. The second employee's left middle fingertip was amputated.
