Incident Narrative

On 8-18-2015, at about 10:00 a.m., an employee was operating a Bobcat machine to remove soil. With soil in the bucket, the employee travelled forward down an incline. The elevated bucket tilted forward and the bucket got stuck. The employee could not get the machine to tilt back and the rear wheels were 1 foot off the ground. The employee's right foot got caught between the back of the bucket and the machine. The top of the employee's right foot bones were crushed, requiring surgery.
