Incident Narrative
On 8/12/2015 an employee who was delivering pavement stones suffered a fractured jaw. The industrial truck that he was operating tipped over while handling a load. The employee was wearing a seat belt and upon the forklift making side impact with the ground the employee suffered a fractured jaw. He was admitted to the hospital for surgery.
