Incident Narrative

On July 28, 2015, employees were attempting to pick up a concrete cure deck bridge. There were three employees (one in the fork lift and one on each side of the bridge holding tag-lines) performing the task. The employees attached rigging on both ends of the bridge to relocate it closer to where the pour will take place. During the pick, the rigging fell out of the hook causing one side of the bridge to fall and hit the employee fracturing the tibia/fibula.
