Incident Narrative
An employee was positioning the lift truck to pick up a tub of parts. He backed into parts tubs behind him and made a rapid forward correction. The quick movement threw him off balance and he fell back. He attempted to steady himself by shifting his left foot, which placed the toe of his foot outside the body of the lift truck. His momentum caused him to pull back on the controls, reversing the lift and causing his left foot to get caught between the lift truck and a tub. The force crushed the steel toe cap of his safety shoe. He sustained bone and tissue damage to the first four toes of his left foot.
