Incident Narrative

On 07/24/15, an employee was using a self-retracting safety knife to cut through a zip tie. The knife got stuck, so the employee attempted to dislodge it. When it came loose, the knife struck the employee's left arm, cutting an artery. The employee was hospitalized and required surgery.
