Incident Narrative

An employee was instructing another operator on how to run a task with a new machine set up in place on the autogrinder machine. He used his right hand to point out a piece of glass as material was being fed into the machine on a small conveyor belt. His finger contacted an in-going nip point on rotating gears, and he sustained a right index fingertip amputation.
