Incident Narrative
Two maintenance employees were checking a belt on a CVP (Cold Vacuum Pack) machine and removed a guard to observe the belt slipping. An employee's finger was caught when the machine started automatically resulting in a soft tissue amputation of the third finger on his right hand.
