Incident Narrative
A maintenance employee was working to clear a clog in the bottom of a cyclone. He had gone through the lockout/tagout procedure twice and asked the operator to control run the machine to unjam the clog. The employee attempted to clear the jam inside the feeder when his glove got caught and his left middle fingertip was amputated. He was hospitalized for treatment.
