Incident Narrative

An employee was found lying on the floor inside the Wal-Mart Distribution Center. He had been unloading boxes and materials from a conveyor and loading them into a truck. He was in and out of consciousness and was initially diagnosed as being dehydrated. The employee was hospitalized for further testing.
