Incident Narrative

An employee was delivering a truckload to a customer on 6/30/15. He was shifting freight to the edge of the trailer for customer access by pulling a chain attached to the freight. At approximately 12:05 PM, the chain broke or became detached and the employee fell out of the back of the trailer approximately four feet to the ground. He was admitted to the hospital for in-patient care of three broken ribs and a bruised lung.
