Incident Narrative

An employee was lifting a cylinder plate (measurements: 7.5 inches tall x 59 inches in diameter, weight 350lbs.) off of a work table to make an adjustment. The part dropped, striking her left foot and resulting in a fracture of the big and second toe, as well as an open dislocation to the third toe. She required surgery.
