Incident Narrative

An employee was changing a fuse in a 440v electric panel when the fuse engaged, causing an arc flash that shorted out the panel and sent out a ball of fire. The employee received first and second degree burns on his face and hands. The employee was wearing safety glasses at the time of the incident.
