Incident Narrative
An employee was operating the Reed Thread Roll Machine with the guard removed. He pressed the button to operate the machine and it did not cycle. He then stuck his left hand into the machine to diagnose the problem and his left index finger was caught in the machine resulting in an amputation above the first knuckle.
