Incident Narrative
An employee on a forklift reached out with her right hand to hit a button to activate the dock door. Her foot slipped off the brake of the forklift. The forklift rolled forward and the employee's arm was caught between the upright on the forklift and a bollard post. She sustained a laceration to her forearm that required stitches and overnight hospitalization.
