Incident Narrative

An employee was standing on the third step of an eight foot step ladder pulling a cable through a conduit with a fish tape. The fish tape popped off the cable and the employee fell off the ladder and struck the ground. The employee was hospitalized with a fractured pelvis and chipped vertebrae.
