Incident Narrative

An employee was cutting the banding on a bundle of 5 3/4" steel on dual hoists and a basket hitch on each end using a manual banding cutter and crushed the tip of his pinky finger between two pieces of steel, resulting in the amputation of his fingertip. He was wearing cut level 4 gloves and sleeves, hardhat, safety glasses + side shield, and steel-toed metatarsal boots.
