Incident Narrative
The injured employee and another co-worker were loading a roll of poly with a rod onto the Line 12 packing machine. The rod goes through the center of a poly-roll. The injured employee was setting the rod into the machine and smashed her left index finger between the rod and the packing machine, amputating the left index fingertip.
