An employee sustained middle finger debriding on the left hand when he was engaged in changing the belt between the motor and the gearbox on the log deck and the belt became energized. Lockout procedures were not initiated prior to maintenance work being conducted. The employee was taken to the hospital and admitted for surgery to reattach skin.
