Incident Narrative

On or about June 4, 2015, at 11:15 a.m., an employee was crushed by a core box when the hand truck, which was supported by a bucket, shifted and pushed him over onto his back while he was still holding the box. He fractured his right hip and was hospitalized for treatment.
