Incident Narrative

An employee was feeding paper into a conveyer belt to be shredded. He saw some paper under the belt and reached for it. His glove became caught in the conveyer belt, which pulled his left hand and forearm over and under the rollers, resulting in a crushing injury. He was hospitalized.
