Incident Narrative
A bag of ice on a pallet in the walk-in freezer had a tear, and some of its ice cubes fell onto the floor. An employee went into the walk-in freezer to get product and stepped onto one of the ice cubes, causing the employee to slip, fall, and break the right hip.
