Incident Narrative
An employee was pushing a plastic bin containing ice to a banquet area when the bin tipped, causing the employee to lose their balance. As the employee fell, the bin fell on top of the employee's hand, amputating the fingertip between the floor and the bin. The employee was hospitalized.
