Incident Narrative

On or about May 12, 2015, an employee was operating a standup reach truck under the direct supervision of a Hubbell, Inc., manager. While driving backwards, he lost control of the vehicle; the "dead man" pedal/switch was not engaged. The employee tried to jump off the moving vehicle, and his left foot got caught between the side of the vehicle and an I-beam. He sustained severe injuries to his foot and was hospitalized.
