Incident Narrative

An employee was placing cardboard into the cardboard compactor chute while standing on a work platform with railings. The employee stepped through the railings to step on the cardboard and push it down further into the chute. The compactor was engaged at the time. The employee's leg got caught and became entangled in the moving cardboard. The employee suffered a broken leg and required hospitalization for surgery. The leg had to be amputated at the knee.
