Incident Narrative

An employee was cutting a piece of scrap steel wire using a propane/oxygen torch. The wire was placed on top of a scrapped hydraulic cylinder that contained a residual amount of hydraulic fluid. While cutting the wire, the employee cut through the cylinder, igniting the hydraulic fluid. The flaming fluid spread to the employee's uniform, which also caught fire. The employee suffered second and third degree burns on his back, stomach, and right calf.
