Incident Narrative

An employee was threading up a draw block on machine D21 (drawer) in jog mode. During the task, the employee was putting a chain on the draw block to pull wire through it. The employee had their hand under the chain to check the tension. The chain became tight and flipped up on the draw block, pinching and injuring the employee's finger.
