Incident Narrative

Two employees were using a forklift to lift a chemical tote bag that was being used to blend chemicals together. A chemical reaction occurred, causing the chemicals in the tote bag to spill out over the top of the tote and onto the employees who were underneath. Both employees were hospitalized with chemical burns.
