On April 20, 2015, at about 1400 hrs, an employee walking on a concrete floor while conducting an inspection of work in progress, stepped on a covered hole that gave way, and fell 10 feet to the concrete surface below. The employee sustained two broken hips, a broken knee and two broken ribs.
