Incident Narrative

An employee working on the sanitation crew at the Further Processing Plant pushed the stop button on a breading machine after removing parts from the upper portion of the machine. The employee then placed his foot into a horizontal auger while climbing down from the machine, causing all five toes on his right foot to be amputated.
