Incident Narrative

On or about 3/29/15, at 2:00 PM, an employee working in the meat department was helping a customer that requested chicken. The employee proceeded to cut the chicken with a band saw when it slipped and amputated the tip of her right thumb. She was hospitalized to reattach the thumb tip.
