Incident Narrative
Employees were lifting a BOP using a forklift to work on the bottom of the BOP. While the BOP was suspended by the forklift, the injured employee placed his hand on the forks. The BOP rolled backwards, catching and amputating his left pinky fingertip between the heavy equipment and the fork.
