Incident Narrative

An employee had just changed the upper inner tool on a powdered metal press. The upper inner tool was not properly aligned with the lower die. When the employee cycled the press, the upper outer tool shattered and created shrapnel, which struck the employee in the neck. The employee was hospitalized for surgery to remove the shrapnel from his neck.
