Incident Narrative

On March 17, 2015, an employee fell off of the fixed ladder on the back of a truck while trying to pull up a 5-gallon bucket of concrete additive. The employee fell backwards, struck another employee standing at the bottom of the ladder, and landed on his right shoulder. He suffered a dislocated shoulder and was admitted to the hospital for a possible broken rib and lung perforation.
