Incident Narrative

An employee was installing a sign while working from an extension ladder. He reached behind the ladder to stick vinyl onto the wall. He slipped and fell approximately 8 feet from the ladder onto the ground, sustaining a concussion, fractured left leg, and cut on his right knee.
