Incident Narrative

An employee passed a barricade in order to help seal an oil leak on the top cover plate of the transformer. While retrieving material that was dropped from the top of the transformer, the employee stepped into a hole in the grating and fell approximately 3 feet, landing on an I-beam. The employee was admitted to a hospital and treated for broken ribs.
