Incident Narrative

An employee was sweeping around the MTB Machine. He bent over at the base of the conveyor and backed into a rotating shaft sticking out of the machine. The shaft caught his jacket/vest and pulled his left arm into it, resulting in multiple fractures. The machine was not shut down at the time.
