On or about 2/1/2015, an employee was operating a stand-up lift truck. His foot slipped off the back of the lift truck and was caught between the back of the lift truck and a wooden crate. The employee was trying to lift his foot off the brake pedal to stop the lift truck but did not completely do so. The employee suffered a broken left foot.
